In order for a Club or Recreational League registered with OVA to obtain a certificate of insurance, the Club or Registered League contact (as listed in MRS) must fill out the Insurance Request Form, which can be downloaded below (for fillable form, please open in Adobe).
2021-2022 insurance requests can be submitted to the OVA starting August 1st, 2021 and they will be forwarded to the insurance company to be processed as soon as possible. Once issued, 2021-2022 Insurance Certificates will be valid up until October 31st, 2021. Once the insurance policy is renewed for the upcoming year, the updated certificates of insurance will be sent to the club contacts. All requests must be sent to firstname.lastname@example.org.
To make a claim, please report the incident directly to Ontario Volleyball. Written notice of injury on which a claim may be based must be given to the Insurer within thirty (30) days after the date of the accident.
