The Ontario Volleyball Association Insurance Program is available to fully registered members of the OVA. The OVA Insurance Program outlines general liability and sport accident coverage.

Certificate of Insurance

In order for a Club or Recreational League registered with OVA to obtain a certificate of insurance, the Club or Registered League contact (as listed in MRS) must fill out the Insurance Request Form, which can be downloaded below (for fillable form, please open in Adobe). 

2023-2024 insurance requests can be submitted to the OVA starting August 18th 2023 and they will be forwarded to the insurance company to be processed as soon as possible.  Once issued, 2023-2024 Insurance Certificates will be valid up until August 31st, 2024. All requests must be sent to insurance@ontariovolleyball.org. 

Making a Claim

To make a claim, please report the incident directly to Ontario Volleyball. Written notice of injury on which a claim may be based must be given to the Insurer within thirty (30) days after the date of the accident.
