In a letter dated September 28, 2021, Dr. Karim Kurji, the Medical Officer of Health for the Regional Municipality of York has announced that owners/operators of facilities located in York Region, where organized sports are played and/or practiced must require any person 12 years of age or older to provide proof of identification and full vaccination status at the point of entry. This rule comes into effect as of Friday October 1, 2021, at 12:01am.

Inquiries about these instructions should be directed to York Region Public Health at: yr.corporate.communications@york.ca

The Instructions, including any revisions to these instructions, are available on York Region’s website at www.york.ca/COVID19

Please check with your facilities to find out if as a permit holder they are requiring your Club to confirm proof of  vaccination and identification at point of entry.

Please review the OVA FAQ from the September 20, 2021 Town Hall Session to learn about what documents can be used for proof of identification and vaccination, and what Clubs can and cannot do when collecting proof of vaccination and identification.

Please see below to access the Vaccination tracking template that Clubs can use.
