Ontario Volleyball requires mandatory police screening (PRCs) and vulnerable sector screening (VSSs) checks of all individuals in leadership roles who work directly with athletes.   This includes all Staff, Directors, Chairs, Coaches, Managers, Officials, Chaperones and persons who manage finances.

Ontario Volleyball requires each person to submit a PRC and VSS check upon registering with the OVA for the first time. There are two options to obtaining your PRC and VSS check: you may use the online services of Hire Performance or visit your local Police Department to obtain the proper documentation. Please check local police department websites for paperwork and procedures that have to be completed.

PRCs and VSSs shall remain valid in subsequent years, provided the individual remains a member in good standing, does not let their membership lapse and self-declares through signature or a check of the appropriate box while registering that they have not committed a crime since submitting their initial VSS and PRC. In the event that an individual does not register with the OVA for a minimum of one year and then re-joins, they shall be treated as a "new" member and must re-submit all documentation as per policy.

Officials - Officials must submit a copy of their original PRC and VSS check to the OVA office, c/o Alishia Lidums

Coaches - please submit a copy of your original PRC and VSS to your designated Club Contact person who will then forward a list of all coaches who have completed their PRC and VSSs to the OVA office, c/o Alishia Lidums

OPTION 1 

Hire Performance Inc. is a Canadian owned and operated background screening company specializing in providing a variety of services including reference checks, criminal and credit checks, driver’s abstracts and pre-employment personality testing tools. Hire Performance has created a quick and convenient way for members of the Ontario Volleyball Association to complete criminal record and vulnerable record checks. 

STEP 1.
As of July 1st, 2007 the cost of the criminal and vulnerable record check is $30.00 CAD plus GST. All purchases are made online through Hire Performances Secure online purchase system. American Express, VISA and MasterCard are accepted. 

STEP 2.
Once payment has been made you will receive information in your email where to access the online forms. Two forms must be filed out; one for the Criminal Record Check and one for the Vulnerable Criminal Record Check.  Once the online release forms are completed, please print the forms, sign where indicated and fax to 416-426-7109.  Each document requiries 2 signatures.  Be sure to submit with all 4 signatures.  Please note: On Form C (Criminal Information Release Form), the witness area must be signed by the Program Manager at Ontario Volleyball. 

• All checks will be kept in a locked location, where only the Program Manager and Executive Director will have access to the files
• Police checks only have to be submitted once to the OVA as long as the member remains in good standing with the OVA and does not let their membership lapse. If membership does lapse, the individual will re-submit their police check upon re-registering with the OVA.

If you are currently employed with a Board of Education, please forward a valid copy of a previously obtained police check  to the OVA. A request for a new police check is not necessary.

Peel Region and Toronto Coaches and Officials

If you live in Peel Region or in Toronto, the Volunteer Criminal Record Search Forms must be obtained from the OVA office before going to the Peel Region Police Department. These forms are not available to download online; original copies must be taken to the Police Department. Please contact Alishia Lidums to obtain the proper forms.

York Region Residents

Please visit www.yrp.ca and go to the On-Line form section. Download and print off the VULNERABLE SECTOR SCREENING form. Once this has been filled out, you must mail or bring the document into the OVA office to obtain the organization representatives signature. The OVA will return completed forms to the applicant. When you have received your signed form by an OVA office staff person, you must bring the completed form in person to one of the York Region Police Department locations (which is listed on the form) along with two pieced of ID, one being photo ID.   Turnaround time is approximately 2 weeks.
