The Ontario Volleyball Association Insurance Program is available to fully registered members of the OVA.  The OVA Insurance Program outlines general liability and sport accident coverage.  To find out about details about coverage please view the Ontario Volleyball Insurance Program document listed below.

Certificate of Insurance
In order for a club or recreational league registered with OVA to obtain a certificate of insurance, all requests are processed through the OVA office.  Please forward your request to This e-mail address is being protected from spam bots, you need JavaScript enabled to view it .

***Please note: Insurance certificates will ONLY be requested on Tuesday and Thursday of each week (beginning September 8th, 2008).  Certificates will take 2-3 days to be issued***

The following information is required along with your request:

1.  Name of the school board(s) & school(s).
2.  Name of the contact person.
3.  Email address or fax number for the contact person (email preferred if available).
4.  Name of the club. 

Out of Province Coverage

Claim forms may be obtained from the OVA office.
