The Ontario Volleyball Association Insurance Program is available to fully registered members of the OVA. The OVA Insurance Program outlines general liability and sport accident coverage.
Certificate of Insurance
In order for a Club or Recreational League registered with OVA to obtain a certificate of insurance, the Club or Registered League contact (as listed in NRS) must fill out the Online Insurance Request Form (for fillable form, please open in Adobe). 2017-2018 insurance requests can be submitted starting July 1, 2017 and will be valid up until May 31, 2018. All requests must be sent to email@example.com and processed through the OVA office.
*All Clubs and Leagues must be registered in NRS for insurance to be valid.
The following information is required to be filled out on the Insurance Request Form:
- Name of the school board(s) & school(s). Please use full school board names, no acronyms!
- Name of the Club contact person.
- Email address for the Club contact person.
- Name of the OVA Club.
Please allow fourteen (14) business days for your request to be processed once it has been received by the OVA office.
If you do not require any additional insured parties to be added to the certificate, a generic copy of the OVA’s confirmation of insurance can be found here.
Making a Claim
To make a claim, please report the incident directly to Ontario Volleyball. Written notice of injury on which a claim may be based must be given to the Insurer within thirty (30) days after the date of the accident.
